NFL Brain Degeneration Study: One in Four Former Players Affected
At least 25% of former NFL players suffer from chronic traumatic encephalopathy (CTE), a progressive neurodegenerative disease linked to repeated head impacts, according to a scientific study published in the British Medical Journal and detailed by the Guardian.
The research, conducted by institutions including Mass General Brigham and Boston University, examined the brains of deceased former professional football players between 2016 and 2021. Out of 878 former players who died during this period, 235 families donated brain tissue for scientific analysis. Neuropathologists confirmed signs of CTE in 215 of those samples, establishing that 24.5% of the studied cohort exhibited the disease at the time of death.
Understanding the Scope and Post-Mortem Diagnosis
Because CTE can currently only be definitively diagnosed post-mortem through brain tissue examination, measuring its true prevalence across living populations remains a significant challenge for medical researchers. Editorial writers Tobias Kurth and Richard Riley, who were not involved in the study, noted that while the figures cannot be directly extrapolated to every former player, the data indicates that the overall burden of the disease within the demographic is likely heavily underestimated.

Symptoms of CTE often manifest years after initial exposure, encompassing both cognitive decline—such as severe memory loss—and behavioral changes including aggression, impulsivity, depression, anxiety, paranoia, and suicidal ideation.
Amateur Football and Cumulative Impact
Professional competition in the National Football League represents only a portion of an athlete’s total exposure to head trauma. According to Daniel Daneshvar, chief of physical medicine and rehabilitation at Mass General Brigham and an associate professor at Harvard Medical School, the majority of concussions and sub-concussive hits sustained by players occur well before they reach the professional ranks.

“The most head impacts experienced by these former NFL players did not happen at the professional level,” Daneshvar stated regarding the cumulative nature of the injuries. “They took place at the college level, in high school, and, in many cases, among youth players. All of these cumulative head impacts contributed to an increased risk of CTE.”
League Response and Ongoing Measures
In response to the publication of the British Medical Journal findings, the National Football League issued a statement emphasizing its ongoing safety initiatives. The league asserted that it “efforts continuously to make football safer, notably by implementing strategies aimed at reducing concussions and head impacts.”
The league also pointed to existing support networks for retired athletes. “The NFL remains committed to ensuring that the NFL community has access to a robust and expanding set of resources to improve physical and mental health,” the league stated, encouraging former players to utilize available diagnostic and treatment programs if they have health concerns.
While the NFL formally acknowledged the link between football and CTE in 2016 and subsequently instituted stricter concussion protocols alongside settlement funds for former players, ongoing research continues to press both professional and amateur organizations to address head safety at every level of the sport.
