The Ebola epidemic in the eastern region of the Democratic Republic of the Congo has become the second-largest outbreak on record, with 1,850 deaths reported among approximately 4,000 cases, according to data from the Congolese government and the World Health Organization. As of August, health officials report that the fatality rate exceeds 40%, driven by the fast-spreading Bundibugyo strain and severely complicated by ongoing regional conflict and delayed international funding.
The Scale and Speed of the Bundibugyo Outbreak
The outbreak, officially declared on May 15, has outpaced previous timelines for comparable flare-ups. According to the Africa Centres for Disease Control and Prevention (Africa CDC), the Bundibugyo strain has killed more than five times as many people as previous outbreaks of this specific variant over a comparable timeframe. During the major DRC epidemic that ran from 2018 to 2020, it took more than ten months to reach a similar death toll.
“I am not in position to tell you that we totally control this epidemic today,” Jean Kaseya, director general of the Africa CDC, stated in late July. Médecins sans frontières (MSF) deputy medical director reinforced that assessment on August 5 during a joint official visit with the WHO director-general to the eastern DRC, noting that the epidemic shows no signs of slowing down.
Security Crises and Humanitarian Hurdles in Eastern DRC
Efforts to contain the virus face severe operational bottlenecks due to persistent violence across the eastern provinces. In Ituri, identified as the epicenter of the current outbreak, repeated attacks by Allied Democratic Forces (ADF) from Uganda disrupt medical coordination and surveillance teams. Meanwhile, the neighboring North-Kivu province remains partially outside Kinshasa’s control following territorial gains by the M23 armed group.
These conflicts have displaced millions of residents toward Uganda, Burundi, and other regions within the DRC, creating dire humanitarian and sanitation conditions. Epidemiological surveillance suffered from insufficient resource allocation at the outset of the crisis, leading to delayed confirmation of the virus’s circulation. According to MSF data from Bunia, 90% of admitted patients were not previously tracked as contacts, while only 59% of contacts have been successfully traced across Ituri province as a whole.
Africa CDC estimates indicate that approximately 40 contacts must be monitored for every confirmed urban case. This standard requires tracking roughly 134,400 individuals, whereas current tracking reaches only 17,500 people, or 13% of the target. Personnel shortages and security risks leave roughly one-fifth of registered contacts without regular checks, and health data shows that 60% of fatalities occur within local communities rather than medical facilities.
Vaccine Trials, Antivirals, and International Response
Medical researchers have mobilized clinical trials to counter the uncontained Bundibugyo strain. A first-phase clinical trial evaluating a vaccine candidate against the Bundibugyo strain commenced in August at the University of Oxford, administering doses to an initial cohort of 50 adult volunteers to assess safety. Additionally, the Coalition for Epidemic Preparedness Innovations (CEPI) is funding a separate vaccine development initiative by Singapore-based Hilleman Laboratories.
Pending widespread availability of a specific vaccine for the Bundibugyo variant, the Africa CDC announced on August 6 plans to deploy the Zaire Ebola vaccine widely among affected populations. While the variants differ, health officials note that individuals vaccinated against the Zaire strain typically experience milder symptoms and lower mortality. Clinical evaluations also involve over 40 patients testing a combination of experimental treatments.
Jean Kaseya highlighted plans to expand the use of the antiviral drug remdesivir in the DRC, pointing to successful deployment in neighboring Uganda. According to Kaseya, Uganda’s lower fatality rate of approximately 10% stems largely from the early administration of remdesivir to patients and tracked contacts alike. Global health projections warn that without accelerated containment, the current epidemic could surpass the 2014–2016 West Africa outbreak, which claimed more than 11,000 lives.
Shifting International Funding Dynamics
The response effort faced initial constraints following shifts in foreign assistance. Earlier in 2025, cuts by the U.S. administration affected long-standing health grants provided through the United States Agency for International Development (USAID), straining Congolese health infrastructure.
On August 5, the U.S. This funding enables the WHO and CDC to meet the estimated $518 million price tag for their six-month response plan. International monitors note that despite this infusion, U.S. spending remains below historical levels of health and humanitarian assistance, though the United States remains the leading donor to the response, well ahead of the European Union.
