A comprehensive new medical study published in the British Medical Journal reveals that approximately one in four former professional football players examined post-mortem showed evidence of chronic traumatic encephalopathy (CTE), placing renewed scrutiny on head trauma in the National Football League. The investigation analyzed brain tissue samples from 878 former players who died between 2016 and 2021, finding that at least 25 percent suffered from the degenerative brain disease.
Because CTE can only be definitively diagnosed after death through the microscopic examination of brain tissue, researchers relied on donations made by players and their families. The study looked at tissue samples donated between 2008 and 2021, incorporating detailed demographic data and information from death certificates to evaluate the true scope of head injuries tied to professional football careers.
Study Findings and Prevalence Estimates
Led by Dr. Daniel Daneshvar, the research team emphasized that the 24.5 percent detection rate among the 878 deceased athletes represents a conservative baseline rather than a ceiling. According to reporting from The New York Times, the actual prevalence rate could be significantly higher because 643 brains from the cohort were not examined.
Dr. Daneshvar noted that while the absolute maximum potential prevalence in similar cohorts can approach 97 percent, the true population-wide rate sits somewhere in the middle. Furthermore, the study established a stark connection between advanced neurodegeneration and cognitive decline, revealing that more than 90 percent of the brain samples classified in Stage IV—the most severe phase of CTE—also carried a concurrent diagnosis of dementia.
Former Tampa Bay Buccaneers offensive lineman Randy Grimes, who played in the league from 1983 to 1992, told reporters that multiple former teammates faced severe neurological struggles. Grimes pointed to former colleagues such as Tom McHale, who died at age 45, and John Bruhin, who died at age 57, noting that the toll of the sport remains a heavy reality for many alumni.
The NFL Response and Safety Measures
In response to the publication, the league issued a formal statement defending its ongoing efforts to protect player health and modify on-field rules. League officials highlighted investments in modern protective equipment, stricter protocols for managing concussions, and expanded healthcare resources.

“The NFL strives continually to make football safer, including through the implementation of strategies to reduce concussions and impacts to the head,” the league stated. “The NFL maintains its commitment to ensuring that the community of the league has access to a solid—and expanding—set of resources to improve their physical and mental well-being.”
Dr. Daneshvar suggested that mitigating cumulative head impacts requires structural shifts in how the sport is taught at younger ages, including removing full-contact drills during team practices and rethinking tackle football programs for children.
“We can change that cumulative force without changing the game of football,” Dr. Daneshvar said, pointing toward preventative strategies that target youth and collegiate levels where head trauma often first accumulates.
