As a father/mother/legal guardian or adult player, I provide:

• a recent passport photo,
• a medical certificate bearing the mention “Baseball/Baseball5 and Softball in competition” dating from less than 3 years ago or a “QS-Sport” health questionnaire (Cerfa 15699).

For any payment by cheque, Sport Pass, municipal vouchers, cash or family registrations (10% reduction on the 2^e intra-family license then 5% on the following ones. The 4^e sibling will only pay for the FFBS license.
